Custom Forms In Wikidot
As robust and powerful as Wikidot is, form handling is absent. However, incorporating forms within wikidot is very easy and if you have your own domain it's even possible to place custom styled within wikidot. For an example of a working custom styled form in wikidot visit this page.
The best free form service I can find is available through Google Docs (Free Gmail account required). You can create your form then place it within wikidot using the iframe syntax.
